About St Vincent's Care at Home
At St Vincent's Health Australia, we're redefining healthcare through our Care at Home services, delivering high-quality clinical care where people feel most comfortable: in their own homes and communities.
Our innovative models of care are designed to:
- Deliver high-quality healthcare beyond traditional hospital walls.
- Improve patient outcomes through evidence-based community and virtual healthcare delivery.
- Reduce avoidable hospital admissions and support earlier discharge from hospital.
- Empower patients to recover safely and independently in familiar environments.
- Create a more sustainable and connected healthcare system for future generations.
This is more than a new model of care. It's one of the most significant transformations in Australian healthcare.
Our Medical Care at Home (MCAH) program provides acute-equivalent care in the community, enabling patients to receive hospital-level treatment at home, work or through virtual care pathways.
Working closely with medical officers, Visiting Medical Officers (VMOs), multidisciplinary teams, carers and community providers, MCAH bridges the gap between hospital and home to deliver safe, evidence-based and compassionate care.
As part of St Vincent's Care at Home, MCAH helps patients avoid unnecessary hospital stays, promotes independence, enhances quality of life and ensures continuity of care through innovative healthcare delivery.
The Opportunity - Registered Nurse, Medical Care at Home
This is a permanent full-time, part-time or casual opportunity based in Toowoomba.
Working as part of the growing Medical Care at Home (MCAH) team, you will provide high-quality nursing care to patients receiving acute and post-acute treatment within the community.
You will work collaboratively with the MCAH Lead (Medical), Visiting Medical Officers (VMOs), St Vincent's Care facilities and a range of healthcare professionals to deliver coordinated, patient-centred care.
As a Registered Nurse, you will assess patient needs, develop tailored care plans, provide clinical interventions and coordinate safe transitions of care.
Your work will directly contribute to reducing hospital admissions, improving patient outcomes and supporting people to recover safely in their preferred environment.

- Deliver safe, evidence-based nursing care within home and community settings.
- Support patients to receive hospital-level care in their preferred environment.
- Reduce avoidable hospital admissions and facilitate early supported discharge.
- Coordinate care across multidisciplinary teams and community providers.
- Promote patient independence, wellbeing and positive health outcomes.
- Contribute to service excellence, innovation and continuous improvement initiatives.
- Help shape the future of community-based healthcare delivery.
Key Responsibilities:
Clinical Care Delivery
Deliver high-quality, evidence-based nursing care including assessment, care planning, treatment, review and education for patients and carers.
Advanced Nursing Interventions
Provide nursing interventions including IV therapies, complex wound care and monitoring of patients with complex clinical conditions.
Care Planning & Coordination
Assess patient needs and formulate individualised care plans that promote safety, function and independence while coordinating care across services.
Patient & Family Engagement
Communicate effectively with patients, families and carers to support informed decision-making and active participation in care.
Clinical Escalation & Safety
Recognise and promptly escalate out-of-range observations and clinical deterioration in accordance with clinical guidelines and organisational protocols.
Multidisciplinary Collaboration
Work closely with medical officers, allied health professionals and community providers to ensure continuity of care and positive patient outcomes.
Quality & Documentation
Maintain accurate clinical documentation and contribute to quality improvement initiatives, case reviews, safety huddles and service excellence activities.
About You
You are a compassionate and highly skilled Registered Nurse who enjoys working autonomously while collaborating within multidisciplinary teams.
You are passionate about delivering patient-centred care and motivated by the opportunity to help transform healthcare delivery in the community.
You thrive in fast-paced environments, demonstrate sound clinical judgement and are committed to achieving outstanding outcomes for patients receiving care at home.
You Will Bring
- Current registration as a Registered Nurse with AHPRA.
- Minimum 3 years post-graduate nursing experience.

Employment Type: Fixed Term Contract (6 months)
Hours: Full Time
Location: Werribee Mercy Hospital
Upload with Application: Resume and cover letter
About the opportunity
This is a rare executive leadership opportunity to lead the opening of one of Victoria’s most significant Emergency Department expansion projects.
Werribee Mercy Hospital is preparing to open a landmark Emergency Department expansion that will double the department's capacity, increase treatment spaces from 33 to 67, and enable the hospital to care for an additional 25,000 patients each year.
Reporting to the Executive Director – Werribee Mercy Hospital, the Executive Director – WMH Emergency Department Expansion will hold single-point executive accountability for the readiness, safe opening and early operation of the expanded Emergency Department.
This is not a project delivery role. Dedicated workstream leads and a Program Director are responsible for delivery across workforce, infrastructure, commissioning, model of care, digital systems, industrial relations and change management. As Executive Director, you will provide strategic direction, executive oversight and decision-making, ensuring all readiness standards are achieved, and the expanded Emergency Department opens safely and successfully.
Working closely with Mercy Health executives, senior medical and nursing leaders, government stakeholders and system partners, you will drive alignment across the organisation and provide confidence that the expanded service is operationally, clinically and culturally prepared to meet the needs of one of Australia’s fastest growing communities.
This role will:
- Provide executive leadership and accountability for Emergency Department readiness, opening and transition to business-as-usual operations
